For professionals
Refer a child.
We take it from there.
One short referral form, one document that helps, and a clear line back to you. Anyone can refer with a parent’s consent: schools, pediatricians, Health Home care managers, county staff, community leaders.

Who refers
No wrong door.
Schools
The child you keep thinking about
A teacher, principal, counselor or school clinician can refer with the parent’s consent. We work in the building alongside you.
Pediatricians
The family that needs more than a visit
Your written recommendation opens four of the six services immediately. Without it, our clinician evaluates first. Either way, refer today.
Care managers and county
Health Home, DSS, community leaders
Send the basics. We confirm receipt, keep you in the loop with consent, and never leave a referral unanswered.
The one document that helps
The LPHA memo, explained.
Four CFTSS services need a written, signed and dated recommendation from a Licensed Practitioner of the Healing Arts (an LPHA) with an NPI: practical support (CPST), skill-building (PSR), parent-to-parent support (FPSS) and youth peer support (YPST).
Counseling and evaluation (OLP) needs none. So if a family has no memo, our own OLP clinician evaluates the child and writes the recommendation. A referral is never blocked by paperwork.
If you are an LPHA and want to send one with the referral, New York State publishes an example memo. See the state’s example.
What we send back
- Confirmation that the referral was received, within one business day.
- With the family’s consent, updates on intake and the plan.
- For schools: collaboration in the building, with the parent’s written permission.
Referral form
A few minutes, then it is ours.
The preliminary referral form takes the child’s details, the service you have in mind, the reason, and insurance. Put your own name and role under “person completing this form,” and make sure the parent or guardian knows about the referral and agrees to it.
